In my practice, dry needling is never a stand-alone fix. I use it to settle a tight, sensitive muscle enough that you can move and exercise more comfortably, so we can get on with the active rehabilitation that produces lasting results.

It’s a safe, well-tolerated technique in trained hands, and it can be a genuinely useful part of treatment for muscle-related pain. Here’s a clear, practical look at how it works and how I use it.

Dry needling is a physiotherapy technique where thin, sterile needles are inserted into tight or sensitive muscle areas to help ease pain and tension. I use it as one part of a broader treatment plan, to make movement and exercise more comfortable, rather than as a treatment on its own. Most people tolerate it well, and it’s typically used for a limited number of sessions alongside your rehabilitation.

What Is Dry Needling?

Dry needling is a technique some physiotherapists use to help with muscle pain and tension, by inserting fine, sterile needles into specific muscle or “trigger-point” areas, tight, sensitive spots that can be linked to pain and restricted movement. The needles are solid and very thin, and nothing is injected, which is where the word “dry” comes from.

I use dry needling as one option within a broader plan, never as the whole plan. Used well, it can settle a stubborn, sensitive area enough that moving and exercising become easier.

How is it different from acupuncture?

People often assume dry needling and acupuncture are the same because both use needles, but they come from very different places. Acupuncture is rooted in traditional Chinese medicine and the idea of energy flow along meridians. Dry needling is based on modern anatomy and how muscles and pain behave, and targets specific muscles and trigger points. The tool looks similar; the reasoning behind where the needle goes is not.

How does it work?

Dry needling is thought to work by calming a tight, irritable muscle and easing the pain coming from it. It’s not about “flushing out toxins” or physically breaking up knots.

What I see in practice is simple: a guarded, tender muscle tends to relax and feel less sensitive for a while, which makes it easier for you to move and to do your exercises. That’s exactly how I use it, to open a comfortable window for the active work that drives your recovery.

Benefits and Uses

Dry needling can help ease muscle-related pain and tension and make movement more comfortable, which is why I use it as a supporting part of treatment. Research supports its use for short-term relief of muscle pain.1

I mainly use it for tight, tender muscles and trigger points that are limiting your movement, for example in the neck, back, shoulders, or limbs. By easing that tension, it can make stretching, strengthening, and everyday movement more comfortable, so your rehabilitation can progress more smoothly.

Common reasons I reach for dry needling in practice:

  • Tight, overactive muscles that aren’t releasing with other treatment
  • Tender trigger points that are limiting movement or comfort
  • Settling a guarded area so you can move and exercise more freely
  • Muscle-related neck, back, and shoulder pain, as part of a wider plan

It’s worth being clear about what it is and isn’t: dry needling isn’t a cure on its own, and its effects are usually short-term. That’s exactly why I use it alongside exercise and hands-on treatment rather than on its own, as a helpful tool to get you moving more comfortably, not a quick fix.

Who Is Dry Needling For?

Dry needling may be a good option for people with muscle-related pain or trigger-point tension as part of a wider plan, but it isn’t suitable for everyone, and a proper assessment always comes first.

It may be worth discussing if you have muscle-related neck, back, or shoulder pain, or tight, tender areas limiting your movement. From what I’ve seen, it works best for people who are also ready to do the active rehabilitation alongside it.

It isn’t right for everyone. It should be approached with caution, or avoided, and always discussed with your physiotherapist first, if any of the following apply to you:

  • A phobia of needles or a strong aversion to them.
  • A bleeding disorder, or you take blood-thinning medication.
  • A local skin infection, open wound, or rash over the area.
  • A weakened immune system.
  • Pregnancy, this needs individual consideration, so let your physiotherapist know.
  • A metal allergy (for example nickel), so the right needles are used.

This is exactly why an in-person assessment comes first, so I can confirm dry needling is appropriate and safe for you specifically.

What to Expect During a Dry Needling Session

A session starts with assessment, not needles: I examine the area, confirm whether dry needling is appropriate, explain it, and get your consent before anything else. Only then, if it’s suitable, are the needles used.

  • Assessment. I identify the relevant muscles and trigger points and check there are no reasons to avoid needling.
  • The technique. Fine, sterile, single-use needles are inserted into the targeted areas. You may feel a brief twitch, a dull ache, or a short cramping sensation, this is normal and usually settles quickly.
  • Afterwards. Some muscle soreness for a day or so is normal, a bit like after exercise, and a little bruising can happen. I’ll usually pair the session with movement or exercises to build on it.

Is dry needling safe?

Yes, dry needling is a low-risk treatment when it’s carried out by a trained physiotherapist using sterile, single-use needles after a proper assessment. The most common effects are minor and short-lived, such as brief soreness or a little bruising.2

Serious complications are rare, but no needle technique is completely without risk, which is why training, sterile technique, and screening you beforehand matter. That’s exactly why I assess suitability first and only use dry needling where it’s appropriate for you.

Frequently Asked Questions

Is dry needling painful?

Most people feel a small prick as the needle goes in, and sometimes a brief ache, twitch, or cramp in the muscle. It’s usually described as very tolerable rather than painful, though everyone’s experience differs.

How many sessions will I need?

It varies with the person and the problem. I use it for a limited number of sessions as part of your plan, and we review how you respond as we go.

Can it be combined with other treatments?

Yes, I use it alongside exercise-based rehabilitation and hands-on treatment, not on its own.

Will I feel a difference straight away?

Some people notice a change soon after a session, others take a little longer. Because the effect is often short-term, the bigger, lasting gains come from the rehabilitation the needling supports.

Is it safe for everyone?

Can I exercise afterwards?
